Workers Compensation Exemption Form

Hello! Below is our workers compensation exemption form.
We use this form to notify our general contractor, and/or
owner of the project, that we do not have employees and therefore
do not have workers comp insurance. You can have this form
for $9.95, which includes an instant download and unlimited
usage!
If you need the Worker's Comp Exempt form that is to be
submitted to the California Contractor's State License Board,
you must obtain
that at the CSLB website. (If you need the form for a
different state, please contact the contractors license board
