1. Instructions on how to use the forms Saving the forms to your computer: You can save the forms wherever you’d like to save them on your computer, except within your Documents folder. You can: • Create
a file folder on your desktop to hold them Changing the names of the forms: You can change the names of the forms to be whatever you’d like them to be except, be sure to leave the .pdf at the end of the file name or your computer will not know how to open the document. Filling in the forms: These documents are created to be used with the free Adobe Acrobat Reader. The forms are designed so that you’ll be able to TAB from field to field and fill in those fields without changing the alignment of the form itself. 1. Navigate to where you’ve saved the form on your computer and click to open it 2. Using your mouse, roll your curser to the first field and click to “activate” it 3. Fill in the field - for specific information as to what goes into each field please see the sample completed documents included in the download 4. TAB to the next field to fill it in and so on and so on… 5. When you're done filling in the form choose "Save As" and save the form under whatever file name you wish. Should you need to make any changes to the information you've entered just open the form, make the changes and save it again. :) 6. Print

Acrobat program will not allow changes to be made to the boiler-plate
portion of the form. You can make changes to the portions that you've
filled in but not to the boiler-plate portion. 3.
